ENROLLMENT DOCUMENTATION
  • Proof of Residence
    • Mortgage Contract
    • Lease Agreement
    • Power Bill
    • Water Bill
    • Gas Bill
  • Driver's License
  • Birth Certificate
    • Kindergartners must be 5 years old ON or BEFORE September 1st.
  • Social Security Number 
  • Immunization Record 
  • Physical dated within 1 year.

Online Enrollment Application

For New Students:

  • Registration begins with an online application, link below. 

    • Have the following information available: 
      • Emergency contact information
      • Insurance information
      • Medical information

  • To finalize your new student registration, you must provide the follow-up documents listed under Enrollment Documentation.

  • Please call the school for any specific questions. (850) 494-5630

  • Only use Firefox or Chrome browsers. The online application will not work in Internet Explorer

For Currently Enrolled Students: 

  • Withdraw the student from his/her current school of enrollment

  • Go to the new school to complete the necessary enrollment paperwork

  • Bring Proof of Address
    If the school you are attempting to enroll in is NOT your residentially zoned school, you must also have School Choice approval prior to enrolling at the school.

 

For Previously Enrolled Students: 

  • ​​​Go to your child's residentially zoned school to complete necessary enrollment paperwork.

  • Bring Proof of Address
    If the school you are attempting to enroll is NOT your residentially zoned school, you must also have School Choice approval prior to enrolling at the school.
  • Bring Current Immunizations

  • Bring Physical Exam within last 12 months
